New Patient Appointment - $160
Established Well-Child - $130
Established Symptomatic - $100
Procedural Only - $50

We accept all forms of payment at the time of service, but we do not bill insurance.
A “superbill” will be provided along with the patient’s insurance claim submission form for the patient family to submit to the their health insurance carrier for reimbursement.

**Send out labs will be billed by the lab through the patient’s insurance.
**Imaging diagnostics will be billed by the imaging department through the patient’s insurance.
